Referral Form

Dresses are given on a first come, first served basis.

Due to space limitations, each girl may bring only 1 guest.

No children under the age of 12 allowed.

Each girl may only receive one dress unless prior arrangements have been made.

Girls will be assisted in the dress selection process by peers their age or an adult volunteer who have all signed confidentiality agreements.

Adults may not shop in place of a girl unless there is a physical disability preventing her from doing so.
